Complaint Forms

To complain or make a request for information to the BC Ombudsman, use one of the forms below.  Before you fill out the form, we strongly encourage you to read the information provided on this web site regarding what the Ombudsman can investigate.  For information on the kinds of complaints that the Ombudsman can investigate, please .  Once the complaint form is received by the Office of the Ombudsman, you will be contacted within five working days by telephone, fax, or by Canada Post.  The Ombudsman will not reply by e-mail.

Submit a Complaint Online:

The online secure web form is designed to be a brief summary of your complaint (not more than 3000 words). For a more detailed submission please use one of our other forms that you can print and mail or fax to us.
